With USFHP, you receive a fully covered annual eye exam through your Pacific Medical Centers provider. Once enrolled in US Family Health Plan, you will receive a member ID card and provider directory from EyeMed. After you have your fully covered eye exam at Pacific Medical Centers, schedule an appointment at the Pacific Medical Centers Optical Shop by calling (206) 505-1200 or visit an EyeMed vision provider near you.
