COVERAGE THAT MAKES SENSE
US Family Health Plan provides the TRICARE Prime Uniform Benefit which covers a broad range of services - comparable to almost any medical plan in the United States.

TRICARE Prime Benefits include:

  • Comprehensive preventive care following established health maintenance guidelines, including routine physical examinations, mammograms, well baby/well child care, and periodic diagnostic testing

  • Medical emergency care worldwide

  • Substance abuse rehabilitation
  • Hospitalization and surgical care

Portability
There continues to be portability among the USFHP sites for enrollees who are 65 and older. However, because TRICARE Prime does not have enrollment for those 65 and older, there is no portability between USFHP and TRICARE Prime or TRICARE Senior Prime. USFHP is included in the portability policy for TRICARE Prime. This means that USFHP enrollees under the age of 65, who move anywhere in the country, may transfer their enrollment from one USFHP to another or to the TRICARE Prime program in that Department of Defense Health Service Region.

Active-duty families may transfer enrollment as often as needed. Retirees and their family members may transfer twice during an enrollment year, a long as the second transfer is back to the original program. Enrollment anniversary dates transfer along with the enrollment, as well as credit for enrollment fees already paid.

What to do if you move
Before you move, you should contact USFHP Member Services at (206) 621-4090. Our representatives will provide you with phone numbers for the USFHP and/or TRICARE contractors in the area to which you are moving.

If you are moving and don't intend to transfer enrollment to another USFHP or to TRICARE Prime, you should submit your request for disenrollment in writing. Be sure to include the sponsor's social security number, the names of all family members to be disenrolled and the date you are moving (which will be your disenrollment date). If you know your new address and/or phone number, please include that information.

If you are moving and do intend to transfer enrollment to another USFHP, or to TRICARE Prime, you should remain enrolled in USFHP at Pacific Medical Centers. When you arrive at your new location, contact the local USFHP or TRICARE contractor to obtain an enrollment application and tell them you are enrolling under portability. The date they receive your application will be the effective date of your enrollment transfer. They will contact USFHP at Pacific Medical Centers and the two programs will collaborate to transfer your enrollment. The actual paperwork may take as long as 15 days.

Remember that when you are outside the USFHP service area, you have coverage for emergency care and preauthorized urgent care, although no routine or chronic care is covered. That is why timely enrollment transfer is so important.

No duplicate fees
Families with members enrolled in different USFHP or TRICARE Prime programs throughout the country need not pay duplicate enrollment fees. If you have more than two family members enrolled in USFHP add/or TRICARE Prime, you should not be paying more than $460 per year in enrollment fees. The USFHP and TRICARE Prime contractors will coordinate your family's enrollment information.

Out of Area Care
When you are traveling out of the area-anywhere in the world-you have coverage for life-threatening emergencies and urgent care through your emergency care benefit. Although routine or chronic care is not covered out of the area, you can obtain refills of your routine medication through the USFHP mail-order pharmacy.
