Eligibility
You qualify if you are an active-duty family member, survivor, military retiree, or family member of a retiree, are listed in the Defense Enrollment Eligibility Reporting System (DEERS) as eligible to receive military healthcare benefits, and live within our service area. This area includes King County, much of Snohomish County, and parts of Pierce, Mason, and Kitsap Counties. Enrollment
Your coverage begins the first day of the month after we receive your application and enrollment fee, if applicable. If you are enrolled in TRICARE Prime or US Family Health Plan elsewhere, you can transfer enrollment when you move in to the Puget Sound area. If you'd like to add eligible family members, they will be covered the first day of the month after the application and enrollment fees, if any, are received. If an active-duty sponsor of an enrolled family retires, that family must submit a new application and any required fees. If an application is submitted before retirement, there is no lapse in coverage.

ENROLL TODAY
To enroll in the US Family Health Plan at Pacific Medical Centers, you must be eligible:

• Active duty family members, including spouses and unmarried dependent children (until their 21st birthday, or, if they are a full- time student, their 23rd birthday)
• Retirees, their spouses and unmarried dependent children (until their 21st birthday, or, if they are a full- time student, their 23rd birthday)
• Family members of a deceased active duty military member or retiree
• Acceptance for enrollment is dependent on confirmation of DEERS eligibility and payment of applicable enrollment fee

When can I join?
You can join anytime. Enrollment with the US Family Health Plan, in most cases, becomes effective on the first day of the following month, when we receive your application.

Are there enrollment fees?

• There are no enrollment fees for active duty family members
• There are no enrollment fees for Medicare A & B participants. Applicants who show proof that they pay monthly Medicare Part B premiums are exempt from the enrollment fees
• Retirees and their families pay the same enrollment fees required by regional TRICARE Prime programs

The enrollment fees are:

• $230.00 annually for a single applicant or
• $460.00 annually for a family of two or more people

1. Complete the application form
To enroll in US Family Health Plan, simply complete the application form. A Member Services representative will be happy to assist you in completing your form if you need help.

2. Return your completed application
Once completed, return your application along with any enrollment fees.

3. Your coverage begins
Welcome to US Family Health Plan. Your coverage begins the first day of the month after we receive your application and enrollment fees.
