Ambulatory Care Pharmacology

Pharmacists have expert knowledge of medications. Ambulatory Care Pharmacists take this knowledge into the clinic setting, where they provide comprehensive medication management for patients with chronic health issues.

PacMed Ambulatory Care Pharmacists employ a team-based approach to managing health issues such as hypertension, dyslipidemia, diabetes, heart failure, and pulmonary diseases. Your Ambulatory Care Pharmacist works closely with your doctor and other medical experts to coordinate medications for your care plan.

Like the rest of your medical team, the Ambulatory Care Pharmacist wants to help you reach your health goals. With deep training in medication dosing, drug interactions, side effects, and over-the-counter (OTC) medications, a pharmacist is an important member of your clinic care team.

The Ambulatory Care Pharmacist collaborates with your doctor to review your medications, adjust doses, monitor medication therapy, and ensure you are realizing the benefits of your medications without a minimum of side effects. The pharmacist will also help you understand how your medications fit into your treatment plan, and how to make each drug work with your daily life. Your pharmacist also provides education on how to take each drug and what reactions may occur, plus answer your questions.
